What's advance care planning?

Advance care planning is a process to make and document decisions about your future healthcare preferences. By taking these steps now, you can help prevent any unnecessary stress on your loved ones if you are seriously ill or injured.

Completing forms and paperwork valid in California

A lawyer can write an advance directive for you. If you prefer to do it yourself, you can choose one of these options:

  • Download, print, and complete our free advance care directive forms in English [PDF] or Spanish [PDF]
  • Attend our free workshop to complete your advance directives
  • Visit the PREPARE™ for Your Care website to learn about medical decision making, how to discuss your wishes with your loved ones and medical providers, and fill out an online advance directive form
  • Visit the Coalition for Compassionate Care of California website to access resources and download a Physician Orders for Life Sustaining Treatment (POLST) form, which gives patients who are seriously ill more control over their end-of-life care

Free advance care planning workshops

Get expert guidance from a registered nurse and member of Montage Health's palliative medicine team. Attend a workshop to:

  • Start planning, thinking, and talking about what's important to you
  • Select the best way to record your wishes
  • Review any advance planning documents you have to be certain they're current and clear
  • Determine who will be your spokesperson and clarify your wishes
  • Submit your documents into your Montage Health and Community Hospital of the Monterey Peninsula medical record

Your rights, the law, and our policies

Rest assured that Montage Health follows federal requirements to inform you of your rights as a patient, including your right to make decisions about your health known.

When we'll ask about your advance directives

If you're admitted to Community Hospital, we’ll ask whether you have completed an advance directive (also called a Power of Attorney for Healthcare) to appoint your healthcare "agent." (The Patient Self-Determination Act and the Healthcare Decision Law require us to ask this.) Even if you have this document, discuss your healthcare wishes with your physician.

You may change any portion of the Power of Attorney for Healthcare by telling your physician about your new decisions or wishes.
